Composite versus amalgam
Most private fillings are tooth-coloured composite, bonded in layers and cured with a light. Silver amalgam is cheaper and still used on the NHS for back teeth, though its use is being phased down across the UK. Composite takes longer to place, which is most of the price difference.
Why quotes vary so much for the same tooth
Practices price by surfaces — how many faces of the tooth the filling covers. A one-surface filling and a three-surface filling are often quoted as one number on a website, so ask which case the published price refers to before comparing two practices.
The NHS alternative
Fillings sit in NHS Band 2, £76.60, which covers everything in Band 1 plus any number of fillings needed in that course of treatment. If you need three fillings, the NHS charge is still £76.60 — which is where the NHS is at its most competitive.
The faster “bulk-fill” technique lasts as well as the slower layered one, so a practice charging a premium for either should be able to say why.
Pooling nine randomised trials covering 632 restorations found no statistically significant difference in failure rates between bulk-fill and incrementally layered composites, over both short and longer follow-up. Marginal adaptation, discolouration and post-operative sensitivity were comparable too. The practical difference is chair time, not durability.
